- Provides case management to perform vaccine-related outreach and education, including promoting health center services to those in need of a medical home.
- Case Management including family support, healthy lifestyle promotion, addressing health compliance issues, tobacco cessation, social and financial resource referral.
- Provide prompt, efficient, and accurate service to community members.
- Interviews and provides social counseling for clients to help clients be successful in meeting his or her healthcare goals.
- Participate in community outreach and partnerships.
