SCOPE AND PURPOSE OF POSITION:
The Senior Care Manager will serve as supervisor of Pathway of Hope and Health Home Care Management in Oswego County. As Care Manager, SCM will carry a balanced caseload providing Pathway of Hope and HH Care Management to eligible families and individuals. As supervisor, the SCM will oversee the activities of Pathway of Hope and Health Homes case/care managers to assure the provision of high-quality, family-driven and youth guided service that is in accordance with the policies and regulations of The Salvation Army, grant funders, Lead Health Home, and the NYS Department of Health.
Senior Care Manager will be responsible for monitoring data quality across systems, track HH billing, and assist with reports/statistics as request or required for program compliance.
Responsibilities:
ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide case/care management services to individuals and families with mixed acuity needs, including at least two services per month, with at least one of those conducted face to face. Balanced caseload of approx 5 POH + 5 HH cases. Caseload may shift periodically as needed for program continuity.
  • Document case activity, including outreach, assessment, client progress and transition arrangements through comprehensive, family driven and youth guided plans of care.
  • Actively initiate referrals to ensure caseloads for team members are at capacity and support program growth. Oversee intake, evaluation and clinical assessments of prospective participants.
  • Work collaboratively with the corps officers to market programs to the community and strengthen the integration of POH and HH models into local unit activities. May include participation in community collaborative groups or attend networking opportunities.
  • Track, monitor and comply with program billing and monthly statistics reporting activities.
  • HH/Care Management Billing: Ensure that monthly billing standards are met for each enrolled member and that a billing questionnaire is submitted within the month that the activity took place.
  • Pathway of Hope grant reports are completed quarterly or as needed per the grantor requirements.
  • Review documentation by direct reports, monitor regulatory compliance; including monthly chart audits of assessments, care planning and care management documentation
  • Provide and document weekly supervision to direct reports focusing on quality of care, compliance, outreach, enrollment, development of goals/plans of care. Monitor care manager performance and provide professional feedback on improvements needed.
  • Ensure direct reports complete and maintain mandatory and optional training(s).
Qualifications:
SPECIAL SKILLS, CERTIFICATES, LICENSES, REGISTRATIONS:

  • Bachelor’s Degree with three years of experience; Master’s degree with one – two years of experience preferred.
  • Experience as case/care manager and/or in Health Homes capacity preferred. Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services
  • Experience as supervisor preferred; experience coordinating and participating in team settings
  • Solid writing and verbal communication
  • Work effectively and knowledgeably across a broad spectrum of cultural, ethnic, and racial communities
  • Ability to work in an effective and focused manner when self-directed and beyond immediate oversight
  • Ability to deliver family-driven, youth-guided services
  • Requires computer proficiency including word processing skills and the ability to provide culturally competent practice
  • Must have and maintain valid driver’s license that meets The Salvation Army insurance requirements
  • Must have regular access to a vehicle to conduct home visits and outreach services
  • Perform all duties associated with job responsibilities.
