Responsibilities
- Perform patient comprehensive assessments and collaborate with multi-disciplinary team members (LPN, Chaplain, MSW, and HHA) to achieve the patient’s desired outcomes and goals.
- Implement and drive appropriate nursing clinical programs focusing on the patients comfort and quality of life, while attending to their emotional and spiritual needs at the end of life.
- Reports changes in patient condition to appropriate personnel and the attending physician. Provides patient care education to patient and other caregivers.
- Makes home visits to attend to patients at end of life.
- Provides support and comfort to the dying patient and caregivers.
- Participates in bereavement and follow-up services.
- Educates field and office staff in how to assess and address spiritual and emotional needs of patients and caregivers.
- Document timely, accurately, and at the point of care using a tablet.
- Care Collaboration/Coordination with primary and specialty healthcare providers.
- Participate in collaborative Team/Case Conferences. Lead discussion on multi-disciplinary cases as appropriate.
Qualifications
- Active RN License in state serviced
- One year of clinical experience as a Registered Nurse (RN) required; Hospice experience preferred
- Valid Driver’s License
- Intuitive knowledge of technology and comfortable with smartphones, tablets, virtual reality, interactive software and artificial intelligence
- Open and adaptive to change
- Strong Communication Skills
- Passion for learning
