Responsibilities:
- Perform and coordinate field visits with the nursing team and as needed by the patient plan of care and in an effort to support their family
- Train and educate patient, family/caregivers and staff on appropriate usage of medications, DME, and supplies as well as the disease progression
- Ensure concerns are addressed and the patient is comfortable - deliver excellence in patient care and attention
- Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in language the patient and family understand
- Demonstrate excellent interpersonal and communication skills.

Qualifications:
- Degree in Nursing (ADN or BSN)
- Valid RN Texas License
- At least 1-year Hospice experience (preferred)
- Valid Texas Driver's License and Proof of Auto Insurance
- Reliable Transportation
- Valid BLS
- Successful completion of Background Screen including employment verification

Salary dependent upon experience.

Salary: $70,000.00 - $80,000.00 per year
