Overview:
Atlantic Health System is seeking a full-time RN Case Manager for the Case management Department at Over Look Medical Center to work 8am-4pm including 1 weekend shift per month.
Responsibilities:
Essential Functions/Responsibilities:
  • Candidate will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served.
  • Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.
  • Candidate will demonstrate and embrace the PRIDE core values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of their job.
  • Coordinates patients through the acute care hospitalization
  • Manages all aspects of discharge planning/transition process
  • Performs utilization management and quality screening.
Qualifications:
Minimum Education:
  • High School diploma or GED required
  • BSN preferred
Licenses/Certification:
  • Valid New Jersey RN License Required
Minimum Experience Required:
  • At least two years of acute care experience preferred.
  • Case Management, Managed Care and/or Home Care experience preferred.
  • EPIC (EMR) experience preferred.
