Shift Availability for Registered Nurse Assessment Coordinator (RNAC):

Responsibilities of Registered Nurse Assessment Coordinator (RNAC):

• Responsible for coordinating all required resident assessment instruments including MDS, CAAS, and Care Plans for state and federal regulations.
• Responsible for monitoring sections of the MDS for timely completion and accurate documentation.
• Participates in the development of a comprehensive multidisciplinary plan of care for each resident.
• Maintains current knowledge base regarding state and federal regulations relative to the MDS process and prospective payments system.
• Monitors input into each MDS to ensure accuracy and consistency of response as a class of interdisciplinary assessment.
• Maintains smooth, seamless and timely communication adhering to state and federal regulations with the Finance Department regarding the resident's Medicare status, RUG Category, ICD Codes, and HIPPS Codes.
• Reviews each new admission for appropriateness of Medicare coverage.
• Consistently monitors alert charting to ensure it is completed for the CMI submission process for Medicare and Medicaid reimbursement.
• Reviews each new admission for appropriateness of Medicare coverage.

Qualifications of Registered Nurse Assessment Coordinator (RNAC):

• Current license as a Registered Nurse in the Commonwealth of Pennsylvania required.
• Minimum 2 years previous RNAC experience required.
• Excellent customer services skills
• Strong computer skills
• Long-term care experience preferred.
