We are looking for a full time Restorative Care Coordinator

Shift from 10:00AM to 8:30PM and every other weekend.

Job description

1. Greet all admissions – Orient new patients to room at the facility and what to expect during their stay. Asist with unpacking.

2. Proactively check and answer all call lights by doing customer service rounds.

3. Serve as a bridge between 1st shift and second shift with critical patient information updates.

4. Assist CNA's with Laydowns after meal times

5. Update nursing roster as needed to keep current patient level of functions accurate for nursing team

6. Restorative- Assist patients to walk to the dining room and back

7. Cover staff during break times

8. Pack patient belongins the day before discharge with boxes

9. Assist patients to their cars upon discharge

10. Ensure new patients are provided with food upon admission

11. Ensure rooms are organized at all times. Patient supplies should always be properly stored away. Patient equipment should be properly stored away. Such as collapsed wheelchairs and walkers.

12. Pass water

13. Assistance with patients who require feeding

14. Assist with showers

15. Ensure patients are properly groomed daily, and before/after meal times

16. Ensure patients belongings and clothes are organized in their closets

17. Audits as needed

18. Determine individual ADL medical supplies needed

19. Assist on the floor as needed if staffing levels are not adequate

20. Take and report Vital signs
