Day Program Rehabilitation Specialist is a direct service position requiring general knowledge of serious mental illness and specific knowledge and skill in rehabilitation with persons with disabilities. This position involves the provision of Day Program services in the context of volunteer support and both individual and group support of clients attending the Day Program.

Responsibilities:

  • Works with clients to learn or regain skills for successful community living including independent living skills (shopping, cooking, cleaning, budgeting and transportation) and socialization (enhancing social and/or romantic relationships, recreational and leisure pursuits that contribute to community integration) utilizing deliberate and consistent skills training which typically includes staff demonstration, working side by side, facilitating groups, client practice/roleplaying and staff feedback, as well as prompting and cueing.
  • Participates in the Individual Rehabilitation Plan (IRP) process by drafting, re-framing and finalizing IRPs in collaboration with the client.
  • Monitors clients’ medications, ensuring compliance with medical regimen.
  • Coordinates comprehensive care to assigned clients.
  • Maintains accurate, up-to-date documentation of all services in a timely manner. In addition to contact notes, end-of-shift notes, and incident reports, is responsible for monthly progress notes, IRPs and utilization and documentation of appropriate assessment tools (DLA-20s), as directed by supervisor.
  • Supports clients in managing medications and complying with medication regimens.
  • Maintains client privacy in accordance with HIPAA, MD Privacy Laws and Sheppard Pratt policy.
  • Completes assigned training and documentation as required by regulation and accreditation bodies.
  • Monitors and reports any changes in client’s conditions, physical health and/or mental health.
  • Monitors, reports and participates in managing home environments for assigned locations including cleanliness, pest control, health and safety and property management. Responsible for addressing immediate health/safety concerns and reporting cleanliness issues to supervisor. Responsible for helping clean with clients.
  • Supports clients in maintaining rules and basic program guidelines. With guidance from supervisor, provides crisis support/intervention and facilitates appropriate utilization of community support systems.
  • Provide transportation to clients in a personal vehicle and/or agency-owned vehicles including large passenger vehicles as assigned.
  • Assists clients in accessing resources, including making referrals.
  • Provides additional and or relief coverage as needed.
  • Depending on assignment, may carry on-call phone.
Requirements:
  • Possession of a bachelor's degree in related field preferred, or equivalent combination of education and experience in the mental health field. Must have a high school diploma or GED.
  • Relevant work, student or volunteer experience.
  • Valid full status driver's license and safe driving record. Driver's license cannot be provisional status.
  • Possession or use of a four passenger car to be used to assist with client transportation.
  • Must be covered under an active auto insurance policy and a valid copy of auto insurance is required prior to hire.
