is position will work as part of a care team to see patients in their homes or a facility.

Duties
-
Identify
patient and family care needs -
Provide
direct
nursing and supportive
care
for the terminally ill patient -
Recognize, report, and document changes in patient/client condition
Schedule
-
PRN – as needed
-
No Nights
-
Occasional weekends
Qualifications
-
Valid Registered Nurse License
-
BLS
Certification -
Valid Driver’s License
Full vaccination for COVID-19 & annual flu vaccination are conditions of employment
