Guaranteed Hours: 40 a week

Job Description:

We are currently seeking a skilled and compassionate Registered Nurse with experience in home health to join our team in Lakeland, Polk County-Florida. As a Home Health Registered Nurse, you will be responsible for providing comprehensive nursing care to patients in their homes. Your duties will include conducting patient assessments, creating care plans, administering medications, providing wound care, and collaborating with a multidisciplinary team to ensure optimal patient outcomes.

Requirements:

• Active Registered Nurse license in the state of Florida or Compact License
• Minimum of 1 year of experience in home health or similar setting
• Proficiency in IV skills and wound care
• BCLS certification required
• Experience with OASIS documentation
• Valid driver's license and reliable transportation for home visits
• COVID-19 vaccination (Medical/Religious exemptions only) as per facility guidelines
