JOB SUMMARY: Has principle responsibility for the total nursing care, assessment, planning, implementation, and evaluation of each assigned patient. Plans, provides and documents discharge planning, teaching and educates patients, family or others to attain optimal health and continuity of care. Assures that the plan of care is carried out in a safe environment through performance and supervision of subordinate personnel. Prepares patient for exam and/or procedure and assists the physician, evaluates the patients post procedure, and document findings. In the absence of a desk clerk, keep the desk free and clear of clutter, maintain the patient’s records with the appropriate forms and requested records and results and keep them accessible to the physicians. Participate in the daily operation of the department. Attend required in-service, yearly education, and mandatory department meetings. Assist in the collection of data for QAPI. Adhere to hospital policy. Maintain good relationships with other staff, departments, managers, and physicians. Acts as charge nurse of the unit with knowledge of staffing and scheduling needs of the nursing department. OSHA Bloodborne Pathogens Class I

SUPERVISES: RN, LVN, CNA, and other unlicensed assistive personnel.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span. Will possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs. Is able to provide the care needed as described in the unit’s/area’s/department’s policies and procedures.

ETHICS AND COMPLIANCE

Employee performs within the prescribed limits of the hospital’s/department’s Ethics and Compliance Policies. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, or upward through the chain of command.

ESSENTIAL FUNCTIONS

  • Provide direct patient care, evaluate outcomes and adjust the nursing care process as indicated to ensure optimal patient care.
  • Able to perform a focused and head to toe assessment and reassessment. This includes pediatric, adolescent, geriatric, and general patient population.
  • Able to monitor the hemodynamic status of the patient and correctly interpret results.
  • Knowledge of cardiac monitoring, identifies arrhythmia, and intervenes using ACLS algorithm and physician order.
  • Prepare patient for examination or procedure and assist the physician.
  • Maintain accurate and continued nursing documentation including patient history, condition, vital signs, treatment, response to intervention and change of status.
  • Treat patient and their family with respect and dignity. Function as a liaison for the patient between the physician and other health care providers.
  • Teach the patient based on identified learning needs; evaluate the effectiveness of the patients learning. Include the family in the teaching as appropriate. Review written discharge instructions with the patient or responsible party.
  • Accurately triage patients. Explain delays and waiting time.
  • Maintain current knowledge of medications and their correct administration based on age and patient clinical condition. Calculate, prepare, hang, monitor and discontinue IV fluids and medication drips.
  • Completes nursing, pharmacy, equipment, and supply charges.
  • Exercise Professional Nursing judgment with respect to the responsibilities of the nursing unit, other RNs, LVNs and unlicensed assistive personnel.
  • Monitors staffing needs and scheduling for nursing units and nursing staff.

SECONDARY FUNCTIONS

  • Arrange for patient transfer to other facilities, complete required documentation.
  • Respond to Code Blue in another area depending upon situation in the department.
  • Follow departmental policies and procedures, objectives, safety, environmental and infection control standards.
  • Participate in monthly outdating of medications and supplies. Complete daily essential items check list, order and stock supplies according to par level.
  • Behave in a professional manner and maintain good interpersonal relationships with other staff, departments, managers, and physicians.
  • Assist the physician with daily rounds and occasional procedures such as thorocentesis, chest tube insertion, central line placement, intubation, paracentesis, lumbar puncture.
  • Assist with the orientation of new employees in the nursing department.
  • Delegate tasks appropriately and provide supervision to fellow nursing staff and unlicensed assistive personnel allowing opportunities to observe and learn when necessary.
  • Arrange follow-up care for patient including but not limited to nursing home care, home health, hospice, follow up appointments, and other physician specialty consultations.
  • Perform other related duties as assigned or requested.

JOB QUALIFICATIONS:

a) Education: Graduate of an accredited school of Professional Nursing

b) Personal Job Related Skills: Effective oral and written communication skills.

c) Licensure, Registry or Certification: Registered nurse currently licensed by the

State of Texas. BCLS and ACLS. Nurse will have one year to get ACLS if the

nurse does not have when hired.

d) Prior Work Experience: One year (2-3 preferred) experience as a professional

registered nurse preferred.

e) Technical Training: Current CPR certification and ACLS.

f) Physical and Mental Requirements: Duties of the position require extended

periods of standing, walking, bending, stooping, twisting, reaching and

lifting up to 50 pounds. Occasional heavy lifting when moving or supporting

patients who may suddenly become weak, helpless or combative.

SKILLS
Effective communication skills. Ability to define problems, collect data, establish facts and draw conclusions. Good knowledge of emergency nursing and strong ability in performance of nursing procedures.

WORKING CONDITIONS
Clean well-lighted, heated, and air-conditioned area. Exposure to unpleasant sights, smells, infectious diseases, blood and body fluids, and occasionally to radiation possible. Work schedule is 12 hours shifts (7a-7p & 7p-7a). Full time 32-40 every week. Overtime dependent upon the need of the department. Holiday and weekend work required.

PHYSICAL DEMANDS
Prolonged extensive or considerable standing and walking. Frequently required to use hands and fingers to handle or feel objects, and tools or controls; reach with hands and arms; bend, stoop, kneel or crouch; talk and hear and smell. Visual abilities required include close vision, depth perception ability to adjust focus and determine color.
