Registered Nurse – Behavioral Health Home Services Integration Specialist (IS) & Psychiatry

DUTIES for BHH:

- Ensure use of patient registry to link services, identify and manage care gaps and barriers, facilitate communication among BHH Services team members and other providers, coordinate recommendations for adjustments to treatment, reconcile medications, gather diagnostic results, close the loop on referrals or treatment, and conduct panel management.

- Review initial needs assessment, health wellness assessment and finalize health action plan and ensure update every six months.

- Meet standards for face-to-face and other contact with consumers and supports. Ensure use of evidence-based practices in services and education.

- Establish relationships with and act as client advocate and liaison between the behavioral health and medical care teams, and other community resources.

- Track frequent consumer conditions and referral processes and meet with referral providers to overcome barriers and improve communication.

- Engage area hospitals, primary care practices and behavioral health providers to collaborate on care coordination to aid in effective referrals based on screenings and timely access to services.

- Provide staff training to ensure knowledge about available resources for consumers and effective and timely referral to treatment.

- Communicate with other BHH team members to incorporate consumer needs into health action plan.

- Oversee Behavioral Health Home shared caseloads.

- Document according to Human Development Center policy.

- May cross-train to other registered nurse duties and locations.

DUTIES FOR PSYCHIATRY:

- Provide direct and supportive clinical services in the CCBHC/psychiatry clinic including telephone triage, rooming patients (obtaining vital signs), monitoring patients after Suboxone injections, refilling prescriptions electronically, coordinating care with other providers, initiating prior authorizations for medications, assisting in referrals to hospital or crisis stabilization facilities as needed, electronic documentation, and special projects as needed.

- Interviews patients and relatives to gather basic personal, social, and medical data relevant to assessing chemical dependency and related disorders. Assists patients in understanding the nature of drug and/or alcohol withdrawal; what to expect, and what symptoms to report to MD/NP. Assists patients in understanding and accepting the need for treatment.

- Provides informative/educational sessions when necessary. Administers medications to patients and observes individuals for possible side effects. Maintain all paperwork required for regulatory agencies. Conduct bottle recalls/count on patients with take-home medication if indicated. Obtain urine drug screens per facility policy. May cross-train to other registered nurse duties and locations.

MINIMUM QUALIFICATIONS:

- B.A. or A.A. degree in nursing with a current registration.

- Minimum of one-year experience as psychiatric RN, knowledge of psychiatric systems and resources, ability to work well in teams and collaborate with other health care professionals.

- Must be licensed as a Registered Nurse in Minnesota and be able to obtain RN licensure in Wisconsin upon hire.
