This position reports to the Director of Nursing and works as part of a team with the Clinic Manager, clinic providers, and clinic support staff to provide quality, coordinated care to patients. The RN will travel to various clinic sites to provide clinician support, patient case management, patient education, triage, monitoring of patient flow and indirect nursing supervision of the medical assistants. CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health and outreach services to the low income, multi-ethnic, uninsured and underinsured in the communities we serve.


HOURS/SITE:
Full-time, 40 hours/week, Monday through Friday at CommuniCare Health Centers at Davis Community Clinic in Davis. Some evening hours required.

RESPONSIBILITIES:

The following reflects managements definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.

  • Provides and manages patient care under the direction of staff providers, including physical evaluations, assessments, and diagnoses as appropriate. Consult with staff physicians as needed.
  • Utilizes the nursing process (assessment, planning, intervention and evaluation) in patient care and daily activities.
  • Furnishes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions, within scope of practice.
  • Provides direct one-on-one patient education and supervises the provision of patient education by Medical Assistants. Participate in group education as needed.
  • Performs independent face-to-face or telephone triage using written triage protocols.
  • Accepts and correctly transcribes verbal or telephone orders from clinicians.
  • Provides follow-up to patients recalled for abnormal labs/tests.
  • Transmits new, updated and refilled prescription orders to pharmacies on behalf of prescribing providers.
  • Assures efficient patient flow by ongoing assessment of clinician schedule and working with other staff to minimize patient waiting time.
  • Administers injections and diagnostic test/referrals as ordered, including immunizations, completing lab and x-ray requisitions and other services ordered by the providers.
  • Performs diagnostic and laboratory tests including but not limited to audiometry, EKG, urine dips, acu-check, urine HCG, hemocue hemoglobin, etc. Supervises and oversees quality assurance of these testing services performed in primary care clinic.
  • Performs patient care including but not limited to wound care, birth control visits, blood pressure checks, medication refills, INR/Coumadin services, etc.
  • Processes clinical patient information for providers in patient electronic health records (EHR).
  • Participates routinely in quality improvement projects using PDSA model.
  • Assists with coordination of specialty clinics, such as orthopedics, pediatrics, dermatology, telemedicine, etc.
  • Assists with meeting the needs of patients with complex medical problems, including facilitating referrals and follow-up care as needed.
  • Assist with coordination of care to other CommuniCare services, including dental care, behavioral health, diabetes education, etc.
  • Attends clinic and general CommuniCare meetings as required.

MINIMUM POSITION REQUIREMENTS:

Education/Licenses: Accepting New Grads. Completion of an accredited RN Program. Current California RN license and good standing with the Board of Registered Nurses required. Current CPR certification required.

Experience: Knowledge of nursing process, scope of practice and standards of care. Previous nursing experience in a medical office, outpatient clinic or hospital. Demonstrated ability to assume responsibility, to work well on a team and problem solve. Demonstrated commitment to high quality health care for low income populations. Previous experience with continuous quality improvement process and with safety, environmental and/or infection control standards. Experience with or ability to learn utilization of electronic health records.


Special Skills/Equipment: Basic computer proficiency in Windows environment. Effective and flexible communication skills. Bilingual Spanish/English required.

Requires a moderate to high level of mobility, dexterity, and flexibility in order to perform essential functions. The employee is occasionally required to walk, bend, reach with hands and arms, and to lift 25 lbs. Must be able to sit or stand for long periods of time. Close vision is required. Work is performed in a typical clinic environment with moderate equipment noise. Occasional travel to other clinics and locations may be required. Ability to bend and move arms and hands in sufficient range of motion to complete physical assessment and type chart notes.

PAY RATE:

  • The pay range for this position is $44.55 - $60.55. Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
  • May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).
