Preferred Qualifications: Bachelors degree and experience in health or human service programs. The Bureau of Social Care and Community Supports works to integrate health and human services within the Medicaid program. The bureau is composed of two units, Housing and Social Determinants.


Duties Description
"This position will work under the Social Determinants unit. Duties include:
  • Working closely with Waiver Management Unit, the incumbent will assit in implementing components of NY Health Equity Reform -1115 Waiver Amendment.
  • Participate in statewide procurement review and scoring, awardee selection and announcement, contract development and execution, in accordance with State procurement guidelines.
  • Engage Regional Social Care Network (SCN) lead entities, community-based organizations (CBOs), and stakeholder coalition groups in each SCN region in regular planning convenings, workgroup meetings. Liaise between SCN entities and DOH on ongoing strategic activities aimed at understanding community needs and mapping out regional/community priorities.
  • Provide technical assistance and resource materials to network lead entities, CBOs, and service providers.
Review SCN workplan deliverables, monthly written and expenditure reports, vouchers, and reimbursement requests.
  • Research evidence informed strategies to implement SDH focused projects authorized in the State budget.
  • Determine project implementation step(s) (i.e., Promulgating Regulations, State Plan Amendments (SPA), Waiver Amendments, Procurements, Medicaid Policy Change, Guidance Document, etc.) and engage relevant teams and units to initiate project.
  • Utilize project management tools in tracking through various implementation stages, anticipate any potential roadblocks or delays and flag appropriately.
  • Ensure project workplans are up to date and provide timely status reports to supervisor. "
