Sun River Health is currently seeking a full-time remote Population Health Specialist to join our team.

Coordinates the oversight and implementation of organization-wide activities related to Population Health Plan for Sun River Health. This includes new population health strategies focused on improving Sun River Health’s performance on quality and cost reductions on existing value-based care contracts as well as new programs through other funding sources. The Specialist, Population Health coordinates across departments including care management, health informatics and quality in order to achieve success

Essential Duties and Responsibilities for a Population Health Specialist:

  • Develops and conducts staff training on areas related to incident management and use of adverse events reporting systems
  • Collaborates and partners with management to monitor and meet the deliverables set forth by MCO VBP programs. Keeps informed of all MCO VBP changes and updates through participation in relevant meetings
  • Contributes to the development of the Risk Management plan, and annual evaluation; promotes the utilization of tools and reports to achieve desired program goals and outcomes, including the Risk Management calendar
  • Assists in the development of annual goals for the risk management department
  • Has primary responsibility for oversight and management of the Adverse Events Tracking system, including liaising with third party vendor, management of licenses and oversight of need for system revisions and updates
  • Tracks, trends and analyzes adverse events ensuring that follow up is completed and that all adverse events are investigated, reviewed and closed
  • Develops quarterly and annual analyses for presentation to leadership and board committees
  • Schedules, facilitates, and coordinates preparation for risk management root-cause analyses
  • Oversees risk related policy development , including plan for communicating policy/policy changes to the workforce
  • Reviews documentation to promote compliance with internal policies and external regulations regarding areas of risk management responsibility
  • Participates in the development of statistical and qualitative reports on risk management trends and patterns and communicates this information effectively to stakeholders

Minimum Education Requirement: Bachelor's Degree required, Master's Preferred

Minimum Work Related Experience: 2-4 years

Job Type: Full-time

Pay: $65,000.00 - $75,000.00 per year
