We're looking for an Administer of Population Health Strategy to lead efforts to improve Easterseals MORC’ performance on quality measures, find data driven solutions to complex areas, lead the organizations population health initiatives, continually seeking ways to enhance and quantify the value of existing programs and services, as well as providing data and other relevant information to program leadership to make decisions related to the development of new product lines and solutions to address emerging market opportunities. Specific duties include:

  • Leading development, implementation, refinement and monitoring of short and long-term population health strategies and initiatives to improve program performance aligned with Easterseals MORC’ overall company strategy.
  • Collaborating closely with Clinical, Information Systems/IT, Finance, Analytics, Quality/Member Experience, , and other key departments across the organization to mine complex data, provide system related advice, and deliver on identified initiatives.
  • Design and develop ways to incorporate data through IT focused solutions to create different models based on evaluation of statistical populations, internal data and public health data.
  • As related to CCBHC, collaborates with the CCBHC Project Director, program staff and other departments to ensure integrity -input and output of data (i.e. EHR collection site, CQMs, etc). Collaborates with OCHN/PIHP and Michigan Department Health and Human Services – CCBHC for population health, metrics and outcomes.
  • Monitoring status of current and projected performance, analyzing, synthesizing and presenting data for follow up and action. As it specifically relates to the CCBHC, gathers, analyzes and develops reports including, but not limited to:
  • – Clinic Quality Measures and State Measurements Standards
  • Satisfaction
  • Health Disparities Impact
  • Process Outcomes, including oversight of outcomes related to CCBHC identified Evidence Based Practices
  • CCBHC Project Goals
  • Identifies areas of improvement for overall process, flow and services to improve outcomes and address inequities
  • Measure existing program participation and success rates to determine effectiveness
  • Continuously assesses and makes recommendations to improve the completeness and quality of data collected
  • Oversees the CCBHC Performance Assessment, goals and activities incorporating in the CQI Plan

  • Collaborating with stakeholders to identify appropriate provider partners for initiatives that would impact key strategic areas of population health.
  • Utilizing analytical, statistical and programming skills to collect, analyze and interpret large data sets to drive solutions and performance improvement.
  • Acting as subject matter expert on assigned improvement initiatives, including program and measurement proposals/recommended changes. Communicates with leadership and relevant internal staff on proposed changes and their impact.
  • Leading and/or participating in research projects, including the evaluation and development of utilization studies and clinical program outcome studies.
  • Championing our culture of quality by supporting workgroups in improving processes and operations, researching and communicating industry best practices, and recognizing teams and individuals for their contributions to quality care.
  • Collaborate with IT and business units to advance the company’s reporting capabilities and improve accuracy of data by being able to develop and prepare departmental scorecards & dashboards showing high level results and trends.
  • Maintaining relationships and effectively communicate workload prioritization and progress to all interested parties.
  • May travel to various locations throughout Michigan to attend meetings, share information and provide consultation (10% of Time).
  • Demonstrates knowledge of principles of trauma informed care.
