Physician Pod Team Leader - Bonus offerings (House Calls)

We are seeking Physicians who desire the opportunity and experience of delivering quality and compassionate healthcare - within proven care models – to elderly individuals and those with complex medical issues, who are the forefront of everything we do. We care for high risk complex patients in their home setting within a radius of our Medical Group office.

The Physician Team Leader will collaborate with leaders on our Accountable Care Organization quality goals, oversee 3-4 Nurse Practitioners, and have a partnering MA with company car to see patients in their home. Position Description

The Physician Team Leader (POD Leader) acts as a resource/coach impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team.

Qualifications

  • Experience in Geriatrics or complex patients preferred
  • Active/unrestricted medical license and DEA
  • Board Certified or Board Eligible
  • EMR experience

Pay: From $1.00 per hour
