Summary:

*This is a hybrid position requiring 3 days in office. Must be local to Illinois.*

Clarity Clinic is an interdisciplinary group private practice of Psychiatrists, Psychiatric Advanced Practice Providers (PAs, NPs), Psychologist (post Docs), and Therapists. Duties/Responsibilities:

• Conducts psychiatric evaluations, medication assessments, and crisis response to new and existing clients.
• Develops individual service plans based on assessment data, and provides therapeutic interventions as necessary including the provision of on-going medication management services.
• Provides consultation and health education to agency staff, caregivers and clients as it is related to the treatment and medication needs of the center.
• Maintains documentation of services including service plans, progress notes, medication informed consent, and other required information in compliance with the company and third-party compliance standards.

Qualifications:

• Possession of current license to practice as a Psychiatric NP/PA as granted by the State of Illinois.
• Cultivate a climate of trust and compassion for the patients.
• Comply strictly with medical care regulations and safety standards,
• Document medical consultations, intakes, medical procedures and progress notes accurately in a timely fashion
• Issue appropriate prescriptions and administer medicine.
• Inform physicians about any problems or discrepancies that might arise.
• Collaborate effectively with team members of various specializations.

• Must pass a pre-employment criminal background check.
• Subject to credentialing process; must maintain credentialed status, as applicable.
• Must maintain certification status.
• Must maintain licensure status.
