Purpose of Job Position

The purpose of this position is to evaluate and treat patients as indicated for the potential to restore functional deficits and underlying impairments resulting from injury and/or illness.

Position Summary

The Physical Therapist provides competent, courteous, and respectful evaluation and treatment of patients for difficulties with mobility, function, strength and positioning and duties outlined in APT scope of practice and respective licensure laws. The PT communicates and collaborates with families, physicians and other health team members and maintains impeccable, professional documentation of services in the medical record. The PT also conducts engaging in-services and training for facility staff on an ongoing basis in restorative and functional maintenance programs. The PT reports to the Director of Rehabilitation and ultimately to the Administrator. FOC’s PTs model and promote competent, courteous and professional services to internal and external customers to develop and maintain a competitive advantage. FOC PTs maintain career development through continuing education and training opportunities and networking. The successful candidate will have a passion for demonstrating the FOC values of Commitment, Compassion, and Caring in providing therapy to patients.

Delegation of Authority

As a Physical Therapist you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Essential Functions of the Job

  • Evaluate Patients for rehabilitation services
  • Develop effective treatment plans.
  • Treat patients according to the treatment plan of care.
  • Supervise the Licensed Physical Therapy Assistant in the delivery of clinical services and interactions with other staff, patients and family members.

Essential Functions of the Job

  • Communicate with physicians, clinical managers, and other health team members, patients, family and facility staff regarding patient progress, problems and plans.
  • Participate in Plan of Care Meetings upon request and Interdisciplinary Team Meetings.
  • Participate in in-service training programs for other staff in the facility.
  • Record treatment notes, progress notes, provide objective input for discharge summaries, and complete daily billing of services per policy and procedure.
  • Instruct patient’s families and/or facility staff in maintenance programs.
  • Participate in discharge planning.
  • Recommend any needed equipment to the appropriate personnel which would serve to facilitate improved delivery of clinical service.
  • Report any problems with department equipment to the facility personnel.
  • Consistently portray the mission, vision, core values, cornerstones and professional image of Family of Caring, exercise good judgment in the performance of the job.

Required Qualifications

B.S., Masters, or DPT in Physical Therapy
