We are seeking to hire a Physical Therapist (PT) to provide trusted, compassionate, and expert home care to the community.

As a Homecare Physical Therapist, you will work with clients one on one in their home environment focusing on functional mobility, fall prevention, and home safety. You will develop and implement creative and evidenced based individual care plans that will enhance clients’ quality of life in their own home. What You Will Do:

  • Interpret philosophy and corporate objectives related to home care to clients and their families, referral sources and community groups
  • Assess client’s rehabilitation needs and potential to develop a physical therapy program which may include therapeutic exercises, gait training, use of adaptive equipment and management on orthotic and prosthetic devices
  • Develop, implement a patient centered physical therapy plan of care; reevaluate and modify plan as needed
  • Record and report client responses to the therapy regimen to the physician
  • Maintain appropriate clinical records in accordance with corporation policies and procedures
  • Collaborate with appropriate members of the health care team to provide physical therapy as well as other services determined necessary
  • Regularly participate in interdisciplinary case conference and document patient information as needed
  • Document patient care practices and information regarding patient status according to agency policy
  • Instruct clients and family members regarding rehabilitative therapy as part of the plan of care resulting in the client/family assuming increased responsibility for care when appropriate
  • Provide educational programs as required to prepare other corporation personnel to assist in client’s plan of treatment
  • Delegate appropriate tasks/assignments to PTA’s
  • Maintain professional competence by participating in continuing education programs
  • Maintains a productivity average of 4.5 visits a day including completion of all necessary and accompanying documentation within 24 hours
  • Arrive on time for meetings/education
  • Practice nursing within the scope of license set by the NH Office of Allied Health Professionals
  • Engages in respectful communication (both verbal and written) with team members, patients, family members, and the community
  • Adheres to agency wide policies
  • Other duties as assigned

What You Will Need:

  • Proof of up to date Covid-19 vaccination card to present at Orientation or engage in the exemption process prior to hire.
  • Per the CDC, "A person is up to date with their Covid-19 vaccination if they have received all recommended doses in the primary series and one booster when eligible."
  • NH PT License
  • DPT preferred
  • Reliable Transportation
