SUMMARY

Assists in the coordination of care management services in the community for the Personal Care Attendant (PCA) and Community First Choice (CFC) programs, which enable persons in need to access appropriate long-term care. Coordinates the efforts of formal and informal caregivers on behalf of consumers without personal bias. Performs all duties in a manner that fosters the achievement of the organization’s mission to identify choices and provide services to help people of all ages, abilities and incomes to live at home.

KEY RESPONSIBILITIES

  • Supports the community with the coordination of person-centered services. Assists with monitoring the consumer and the plan of care via telephone contact and/or home visits; hospital admissions and discharges; entitlement eligibility; PCA procedure; home modifications and evictions.
  • Assists Care Managers with varied tasks including contacting clients and scheduling initial assessments, sending letters, obtaining paperwork and uploading into the state system, and maintaining a scheduling log.
  • Assists in completing applications, forms and additional documentation as required. Completes Supplemental Nutrition Assistance Program (SNAP) and fuel assistance redeterminations. Assists with entitlement and other applications, such as Medicaid applications and/or redeterminations, T-19, housing, SNF and transportation. Accesses state database to determine consumer eligibility.
  • Triages consumer, family and provider contacts with appropriate follow-up, including checking Care Managers’ voicemails and providing follow-up on behalf of Care Manager when out of the office. Assists the community with the consumer monitoring processes. Completes quarterly contacts or completes billable activities.
  • Houses pending clients, or clients awaiting closure and monitors these clients for changes in status.
  • Maintains regular professional contact with outside agencies and networks, including Allied Community Resources.
  • Supports manager to track data, dashboards and statistics on assignment statuses of work under Care Managers.
  • Serves in a supportive role in billing for assessments and reassessments utilizing CONNEXUS.
  • Participates in and works effectively as part of an interdisciplinary, self-directed team. Participates on internal committees and agency in-service programs as assigned.
  • Mentors new administrative staff as needed.
  • Performs additional related duties as assigned.
Requirements:

Education

  • High School Diploma or equivalent required; Bachelor’s Degree preferred.

Experience

  • Must speak Spanish fluently
  • At least one year of experience in a human service setting preferred.
  • Project management and organizational skills.
  • Strong verbal and written communication skills.
  • Ability to work with clients, families, providers and other staff.
  • Knowledge of community resources preferred.
  • Ability to maintain confidentiality of client and company information.
  • Ability to understand and articulate multiple programs.
  • Familiarity with the needs of a geriatric population is preferred.
  • Working knowledge of Microsoft Office products.
