Patient Navigator Job at Clinical Professional Connections
ESSENTIAL FUNCTIONS/DUTIES
Houston
Conducts patient outreach through high volume phone calls, letters, etc. to inform patients of needed care, to receive screening tests, return specimens, schedule colonoscopy and documents communication.
- Works with patients to identify barriers to care and helps identify resources to address barriers (e.g. transportation, etc.).
- Communicates information appropriately with care teams related to patients needing test kits.
- Works with Registered Nurse Case Manager to establish weekly/monthly assessment goals.
- Collects data to monitor progress of outreach goals, reports monthly
- Assists with various projects, including training and support (e.g. EHR upgrades, clinical training, competencies, etc.).
- Actively participates in Clinical Improvement Workgroup, assists with risk management rounds, committee projects and takes meeting minutes.
- Assists Director of Quality, Nurse Managers and others with administrative functions
- On rare occasion, may perform in direct patient care role to meet patient care needs.
- Displays a caring and helpful attitude when interacting with patients, vendors and fellow employees.
- Strives to build cooperative partnerships, by treating all people both inside and out of the faith community as a customer.
- Ability to establish and maintain an effective working relationship with other clinic personnel.
- Handles confidential information with tact and discretion on a need-to-know basis.
- Follows established policies and procedures for safety, infection and exposure control.
- Performs other duties as assigned.
POSITION REQUIREMENT
Certificate or diploma from an accredited Patient Care Navigator/Technician Office Administrative Assistant
Or Admin Medical Receptionist
Minimum of 1-3 years Office Administration, making phone calls and appointments
Demonstrates verbal and written communication skills.
Excellent customer service skills.
Ability to work collaboratively with a wide variety of people, providers and agencies.
Self-motivated, willing to take the initiative and work autonomously.
Ability to apply critical thinking and assessment skills and EHR System
Strong organizational and efficient time management skills.
Computer skills is a must, preferred would be working with EXCEL and Outlook (Microsoft Office)
Ability to consistently meet department work schedule as designated.
SCHEDULE
Monday through Friday; 8:30 am.—5:00 pm with intermittent varying hours
Compensation Range will depend on Office Admin Skills
Salary range $ 28,000 to $31,000
