YoungMoms works with moms ages 21 and under who live in Southern Chester County, PA. Many of our participants face numerous challenges such as incomplete education, poverty, single parenthood, depression, and a general lack of options and opportunities. Job Summary: YoungMoms Case Manager:

The YoungMoms Case Manager is expected to build strong partnerships with pregnant and parenting teen moms in Southern Chester County, PA. The Case Manager meets with clients to assess needs, situations, strengths and support networks. In addition, the Case Manager helps clients articulate educational, vocational, relational, and parenting goals and create plans to achieve them. The Case Manager connects clients to appropriate community resources and maintains detailed records of client interactions. The Case Manager interacts with volunteers and one-to-one mentors, and attends and provides support at the monthly “Club” meetings and other special events hosted by YoungMoms. The Case Manager serves as a liaison between the organization, community, and other agencies to support and fulfill the overall mission of YoungMoms. This position requires strong problem-solving skills, a tremendous capacity for working with an at-risk population, the ability to multi-task, and fluency in both English and Spanish. While performing the duties of this job, the Case Manager would work in the YoungMoms office in Kennett Square and drive to clients' homes and other service agencies in the community as needed.

Duties Performed/Responsibilities:

  • Promotes the mission and vision of the organization.
  • Helps clients identify goals and develop individualized plans to improve their well-being.
  • Provides intensive case management services to ensure that clients receive the necessary support for achieving individual goals. Makes outside referrals when necessary.
  • Researches, refers, advocates and assists clients in applying for resources and benefits such as food stamps, childcare and healthcare.
  • Provides on-going assessment of client progress toward achieving goals.
  • Schedules and conducts home and office visits with clients.
  • Communicates regularly with mentors in order to monitor the mentoring relationship and provide support, encouragement and feedback to the mentor and client.
  • Corresponds with clients and volunteers via Facebook and other forms of social media as needed.
  • Tracks and documents client progress. Maintains and updates client files and other necessary documentation according to organization policy.
  • Develops relationships with schools, county nurses, and other referral agencies.
  • Responds to crisis situations.
  • Cultivates partnerships with local service agencies and professionals to enhance the program and provide support to clients.
  • Maintains familiarity with Federal, State and local programs to assist clients in reaching their personal, educational and vocational goals.
  • Prepares written status reports regarding the program and/or clients and submits to the Director as required.
  • Evaluates the effectiveness of services provided and strategies used.
  • Notifies Director of any issues, problems or concerns involving clients, volunteers, or any other areas of the program.
  • Assists with training, supervising and communicating with mentors.
  • Attends monthly “Club” and other YoungMoms events and functions.
  • Attends staff meetings and training programs.
  • Performs other duties as assigned by the Program Director or Executive Director.

Qualifications:

  • Bachelor’s Degree required, preferably in social work, human services, or some related field
  • Bilingual in Spanish and English required
  • Experience working with at-risk youth
  • Excellent oral and written communication skills
  • Ability to take initiative, manage multiple tasks and meet multiple deadlines
  • Ability to lead and encourage clients
  • Ability to develop and maintain relationships with referral agencies
  • Strong skills with Microsoft Office Suite
  • Familiarity with Mac-based technologies a plus
  • Have and maintain a valid state driver’s license, maintain automobile insurance coverage and have access to an automobile
