POSITION SUMMARY: The Behavioral Health Provider is responsible for providing Community Living Support (CLS) services to Waiver participants and their families to facilitate the Waiver participant’s achievement of his/her goals of community inclusion and remaining in their home. Services are primarily provided in the participant’s home or appropriate community setting but some telehealth services are applicable. The CLS will deliver interventions targeted at enabling the waiver participant to attain or maintain their maximum potential, while facilitating the Waiver participant’s independence and integration in to the community. The CLS is responsible for accurately documenting services, attending Wraparound Team meetings, and communicating participant progress to supervisors, team members, and the Local Mental Health Authority. All services are delivered in compliance with the YES Waiver Policy and Procedures manual, YAP standards, and all state and federal regulations governing the delivery of service.

*Office is located in Austin. However, primary job responsibilities are in the community and not in the office.*

POSITION QUALIFICATIONS

EDUCATION: A Master’s or Bachelor’s degree in a human services related field, as defined by Texas Administrative Code i.e. Social Work, Psychology, Human Services, Education.

MAJOR JOB RESPONSIBILITIES/ESSENTIAL FUNCTIONS

Program and Fiscal Management: Assist program leadership in management of programs and client services by working authorized hours and following all processes that impact the fiscal success of the program.

Outcomes: Submits all required reports in a clear and legible manner to program leadership within the required timeframe.

Documentation: Ensure that programs are in compliance with federal, state, local and YAP documentation requirements.

Community Connections and System Change: Promote YAP as a needed, valued and credible resource in the community.

Individualized Service Planning: Ensure Individualized Plans of Care are implemented as written through weekly purposeful activities with youth and families.

Staff Activities: Attend and participate in staff related programs or activities and staff meetings.

Training/Computer Skills: Ensure all internal and external training is completed in a timely manner.

Supervision: Ensure there is a face to face/remote meeting on a weekly basis with program leadership that addresses both clinical and administrative elements of the work and builds an atmosphere of accountability and engagement.

PAY RATE:

$21-$25 per hour depending on degree and whether fluent in Spanish
