The Palliative Care Nurse Practitioner provides care through the evaluation and assessment of the health history and diagnoses of medical conditions. You will manage patient health problems by directing and developing the plan of care, prescribing medications or treatments, etc. You will build from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities.

Schedule: PRN (As needed Monday through Friday 8:00 AM to 5:00 PM)

JOB RESPONSIBILITIES:

  • Provides medical care, palliates, and manages terminal illness in compliance with the state nurse Practice Act under the supervision of a collaborating physician
  • Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions, and documenting the findings in the patient's medical record
  • Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
  • Serve as a nursing resource for consultation and education to members of the interdisciplinary team and other healthcare practitioners in the community
  • Make visits to patients for admissions and palliative care
  • Focus on advanced care planning
  • Focus on pain & symptom management to improve quality of life
  • Education around chronic and serious illness/s
  • Develop emergency plans to help keep patients out of hospitals and emergency rooms
  • Collaborate with PCPs and other specialists
  • Other responsibilities as assigned

REQUIRED EDUCATION & EXPERIENCE:

  • Must have a current Nurse Practitioner licensure in the state, in good standing
  • Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program. Accreditation from an approved certifying body for advanced practice nursing as required by State.
  • Must have a Master's Degree
  • Minimum of one year of Nurse Practitioner experience or Palliative Care experience preferred
  • Certification in a specialist area preferred (Palliative Nursing (CHPN), Pediatrics, or Geriatrics).
