Divinity Hospice has an immediate opening for an on-call RN to join our team!
Qualified RN will be compensated for on-call time, per visit rate and travel. Main coverage areas are South Dallas, Duncanville, Desoto, Arlington, Mansfield, Fort Worth, Midlothian, and Waxahachie.
Primary functions are to provide afterhours admissions, nursing care and death pronouncements for clients in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency; and resumes the responsibility for coordination of care.
EXPERIENCE AND QUALIFICATIONS
· Licensed as a professional in the state of Texas as a Registered Nurse
· Two years of experience as a Registered Nurse in a clinical care setting preferred, hospice or home health experience preferred.
COMPETENCY/SKILL REQUIREMENTS
· Knowledgeable of Medicare, regulatory, and standards
· Excellent written and verbal communication skills
· Able to utilize a telephone for internal and external communications
· CPR certified
· Competent in care provision in area of professional licensure
· Strong computer skills – We currently use WellSky (formerly Kinnser) EMR
· Team player
JOB RESPONSIBILITIES
· Provides leadership and oversight of clinical activities to include, but not limited to, patient assessments, individualized care plan development, care coordination, on-site field visits, case management and case conferencing.
· Promotes hospice mission, goals and philosophy.
· Participate in care coordination to ensure proper communication between caregivers, patients, and physicians.
· Provide oversight of appropriate documentation, quality of care, contact with physicians, adherence to the care plan, and evidence of care coordination between disciplines.
· Delegate the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies.
