Job Description:

  • Provide consultation in palliative care, symptom management and supportive care to meet the needs of the patient as requested by the Team Leader.
  • Conduct face to face visits with our hospice patients to determine continued eligibility for hospice services.
  • Consult and coordinate with the attending physician, the hospice medical director and other members of interdisciplinary team to contribute to the patient’s plan of care.
  • Prepare and maintain accurate patient records, charts and documents.
  • Assist with the development of clinical practice guidelines/standards in support of quality palliative care as needed.
  • Develop and update plan of care with the IDT and others involved in the patient’s care.
  • Provide skilled nursing as determined by the plan of care.
  • Educate patient/family and other primary caregivers in appropriate care techniques.
  • Serve as the patient/family advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers and others.
  • Ensure and document appropriate use of medications, durable medical equipment and supplies.
  • Provide appropriate, coordinated care plans.
  • Monitor the recertification/reauthorization process.
  • Report necessary information to the Team Leader and other members of the IDT.
  • Respond to on-call issues as clinically appropriate.
  • Integrate quality improvement results into the established plan of care.
  • Participate in quality assessment and improvement activities as assigned by the Clinical Director and/or Team Leader or QAPI Manager as requested.
  • Participate in proactive patient calls and visits.
  • Develop and achieve professional growth goals and objectives, review with Supervisor on a regular basis.
  • Accurate documentation will be completed as outlined in the guidelines of Grace Healthcare Services, state and federal regulations.

Qualification:

  • NJ licensed registered Nurse Practitioner
  • Masters Degree in Nursing
  • A minimum of one year nursing experience
  • Excellent oral and written communication skills
