Behavioral Health Case Manager
Summary

Reports to the Coordinator of Case Management and works cooperatively with community partners to provide outreach and case management services to persons with mental illness and/or substance use disorders. The ultimate goal of behavioral health case management services is to engage/re-engage persons with needed community resources.
Essential Duties & Responsibilities
  • Demonstrate the ability to provide case management services including: assessment of needs, treatment planning, providing information/referral about available services (within and outside of GESMV), and coordination of care.
  • Performs work in an ethical manner and ensures that persons are treated with dignity and respect. Self-determination of persons must be acknowledged and respected.
  • Compile necessary reports and documentation as required.
  • Provide supervisor with necessary documentation and reports in a timely fashion.
  • Travel within the community to assist persons in securing necessary resources as required. Provide transportation with agency van and maintain adequate liability insurance.
  • Other duties as assigned.
Knowledge, Skills, & Abilities
  • Minimum of a Bachelor’s degree in Behavioral Health related field required and some level of licensure in treatment of mental illness or substance abuse preferred.
  • Education related to and experience working with persons who have experienced homelessness, mental illness and/or substance abuse problems.
  • Experience working cooperatively with law enforcement preferred.
  • Ability to document, record, write information accurately and thoroughly.
  • Must have working knowledge of Microsoft Office products.
  • Must possess excellent interpersonal skills with the ability to relate well with a broad spectrum of people; including consumers and employees.
Qualifications

Must maintain a valid driver's license and a driving record acceptable to Goodwill's liability insurance provider.
Maintain the ability to pass a criminal background check and random drug screens.
