We are seeking a Masters' Level Social Worker. Experience with Home Health Care and the Geriatric population are required.
This is a per diem position, Monday through Friday, with no weekends and no holidays. Pay is $450 per week guaranteed for up to 15 hours of work, $30 per hour for any additional hours. Covered area is valley wide.
Job Duties:
- Accountable for assisting in the assessment and treatment of the patient and developing Client Care Plans.
- Responsible for providing care and referral services to community programs for various Home Health patients in Phoenix and surrounding valley areas.
- Plan for client’s discharge and prepare discharge summaries and instructions.
- Consult with the Physician and other members of the health team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the client's health problems.
- Must have an understanding of meals on wheels, AAA, community transportation, AHCCCS requirements.
