JOB TITLE: Medical Records Clerk

GENERAL SUMMARY OF DUTIES: Responsible for scanning and chart preparation.

ESSENTIAL FUNCTIONS:

· Scan patient information into EMR

· Review medical records and ensure accuracy

· File medical records, reports, and dictation

· Locate and deliver patient charts

· Execute release of medical records following New Jersey State law

· Prepare charts for upcoming appointments

· Document and fulfill patient medical record copy requests

· Fulfill medical records requests from insurance companies and attorneys. Maintain financial records of same.

· Monitor and respond to tasks in EMR to locate charts and results required by staff

· Sort and distribute incoming faxes. Send outgoing faxes.

· Open (if appropriate), distribute, and/or file all paperwork received via US Mail and daily inter-office mail delivery

· Identify and report office supply needs

· Perform other duties as assigned

EDUCATION: High school diploma or GED required

EXPERIENCE: One year experience in an office environment, preferably medical.

REQUIREMENTS: Adherence to all policies and procedures, including confidentiality, safety, performance, attendance, punctuality, and personal grooming and appearance. Ability to read, understand, and follow oral and written instructions. Ability to speak and write in a professional, courteous, and concise manner. Accept and utilize feedback and constructive criticism.

SKILLS:

· Excellent interpersonal, typing, and organizational skills

· Excellent use of computers, printers, telephones, fax machines, and other office equipment

· Ability to multi-task, be detail oriented, problem solve, and prioritize responsibilities

· Work well both independently and as a team member

· Strong knowledge of medical terminology

· Demonstrate efficient use of time and ensure work is accurate and of high quality

· Establish and maintain effective working relationships with patients, colleagues, physicians, and the public

· Successfully manage interactions with challenging or difficult people

PHYSICAL/MENTAL DEMANDS: Requires sitting, standing, walking, bending; hand dexterity for telephone, computer and equipment use; and normal vision, speaking, and hearing. May lift 10 – 25 pounds.

ENVIRONMENTAL WORKING CONDITIONS: Medical office environment. Work may be stressful at times. Interaction with others is constant and interruptive.
