Overview

Manager of Care Management leads interdisciplinary core teams in delivering comprehensive, consistent, timely, effective, and patient/family centric care. The Manager is responsible for the oversight of the staff and daily operations for the Population Health Services Organization (PHSO) Care Management department. Provides training, support to direct reports, encouraging individual and team development. Participates in process improvement: ensuring policies, procedures, and customer-service are constantly monitored, evaluated, and supported. Assist the Director in ensuring regulatory compliance. As part of the management team, the manager assists the director in fulfilling departmental goals.

Salary Range:

$91,902- $137,854; commensurate with experience

Responsibilities

Accountable for resources, quality control and quality assurance procedures and processes.

  • Regularly monitors quality control and quality assurance under the direction of the Director.
  • Ensures cost efficient utilization of department resources by monitoring budgets and expenses developing improved techniques and practices.
  • Responsible for monitoring budget variances and plans appropriate adjustments.
  • Contributes to the budget and capital equipment processes, and other departmental planning.
  • Makes recommendations for capital equipment, space, operations and personnel and both long and short-range plans for the division.
  • Promotes the importance of quality improvement initiatives and improvement programs.
  • Maintains knowledge of regulatory and legal requirements, including but no limited to HIPAA, JCAHO, compliance, and CMS.

Demonstrate and share expertise to ensure best practice for Care Management work teams.

  • Provides leadership in enhancing professional practice of direct reports.
  • Demonstrate decisive leadership abilities.
  • Ensure that all written and electronic documentation as required by regulatory agencies, hospital and department is accurate and complete, maintains strong competencies.
  • Responsible to develop and deploy education and support for student and new employee orientation in alignment with new institutional academic mission.
  • Inquire, educate, seek additional education to maintain self-knowledge and grow best practice trending locally and nationally.
  • Active engagement with Director regarding professional growth and feedback to enhance leadership skills.
  • Maintains fluency in care management practice by actively engaging in direct practice.

Performs human resource functions as outlined in Medical Center policies and in coordination with Director, PHSO Care Management.

  • Interviews and recommends candidates for employment, Works collaboratively with HR Talen Acquisition Partner.
  • Oversees staff schedules; including PBT requests cross coverage, leave of absences for core team. Reviews, edits and approves KRONOS timecards in a timely and efficient manner.
  • Evaluates and counsels employees in a timely manner, develops an annual performance plan for each employees.
  • Develops and maintains performance standards.
  • Participates in corrective action and employee recognition.
  • Actively intervenes to resolve problems and conflicts, creates an environment for the free flow of ideas, opinions, concerns.
  • Ensures adequate orientation for new staff, ensures training programs are current and used appropriately.
  • Communicates regularly and productively with all contacts, keeps Director, and staff informed of necessary information.

Special RN Care Manager Accountabilities.

  • Exemplifies the highest professional standards in Nursing Practice and sets the standard for the core team.
  • Collaborates with area institutes of higher education to support the academic mission of the Network, to include precepting interns and ensuring a robust experience.
  • Encourages continued professional development among team and identifies the same for self.
  • Adheres to ANA Code of Ethics.

Qualifications

  • Bachelor of Science in Nursing required. Masters preferred.
  • Current RN licensure in New York State required.
  • Case management accreditation by a nationally recognized accrediting body for case management (examples: CCM, ACM or ANCC certifications) is preferred.
  • Knowledge of care management standards of practice, insurance structures, Medicare and Medicaid rules and regulations, clinical practice guidelines and quality issues as related to care management is required.
  • Understanding of issues and resources available for the needs of diverse populations, knowledge of gaps in service delivery systems for population served required.
  • Knowledge of team/small group principals and dynamics is required.
  • Demonstrates strong communication (verbal & written) skills required.
  • Promotes a culture of leadership in a group environment required.
  • Excels in training, leading, and motivating teams.
  • Promotes a high degree of morale and positivity within the work environment required.
  • Demonstrates a desire to build honest and trustworthy relationships with all individuals.
  • 3-5 years of leadership experience in a healthcare setting required
  • 3-5 years of clinical experience in a healthcare setting (or equivalent) required.
  • Care Management/Care Coordination experience required.
