Care Circle HH has an IMMEDIATE OPENING for SHIFT CARE LVN's (4-5 hours per shift) for patients in El Monte, CA.

This position is open to START IMMEDIATELY and YOU SHOULD BE ABLE TO ACCEPT and TAKE PATIENTS ASAP.

Care Circle Home Health is a Joint Commission-accredited home health agency servicing various counties in SoCal.

We offer flexible schedules, training, complete PPE’s and competitive pay. We also use user-friendly EMR/web-based documentation and we concentrate patient assignments close to our nurses’ homes to minimize time in traffic.

LVN's : at least 1 year nursing experience preferred, but newly grads are also accepted

Home Health: 1 year (Preferred)

You will need to submit:

• Current California-issued Professional License
• Certificate for Basic Life Support or Active CPR Card
• Driver's License and continue
• Auto Insurance (with the applicant's name on the policy)
• Physical examination within 6 months or "Fit to Work" doctor's note
• PPD or TB/Chest X-Ray test result
• Flu Shot record
• Proof of Covid Vaccine

IF YOU HAVE ALL THE CREDENTIALS AND QUALIFICATIONS, YOU MAY BE HIRED IN A WEEK!

You will be working with our highly qualified Clinical Management Team (RN Case Managers, Coordinators, dedicated QA Team) to provide quality direct patient care, such as:

• ensure quality and safe delivery of home health nursing services
• providing accurate and timely documentation consistent with the plan of care
• providing patient and family/caregiver education and important information relating to diagnosis and plan of care
• actively participating in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the clinician and/or clinical manager.
