Responsibilities
- No on-call requirement!
- Flexible schedules. Work 5 days a week M-F.
- Collaborates and coordinates patient care as a team with the designated RN Case Manager and other disciplines following plan of treatment and patient care plan, focusing on the right care at the right time for the right outcomes.
- Performs visits as prescribed in the physician’s plan of treatment and in compliance with federal, state and county guidelines.
- Implement and drive appropriate nursing clinical programs and initiatives to achieve desired outcomes, including the use of technology and telehealth to advance your patient’s goals and outcomes
- Document timely, accurately, and at the point of care using a tablet.
- Participates in case conferences, summary reports, quarterly record review, annual program review and in-services or special case orientations.
Qualifications
- Active LPN License
- One (1+) year of clinical experience as a LPN required
- Must obtain and maintain Current CPR Certification
- Valid Driver’s License
- Intuitive knowledge of technology and comfortable with Smartphones, tablets, virtual reality, interactive software and artificial intelligence
- Open and adaptive to change
- Strong Communication Skills
- Passion for learning and teaching
