The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a full time opportunity on our day shift for Sacred Heart location at 1555 S. Layton Blvd, Milwaukee, WI 53215.

The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a full time opportunity on our day shift for Sacred Heart location at 1555 S. Layton Blvd, Milwaukee, WI 53215.
Licensed Practical Nurse/LPN Job Responsibilities:
- Work in collaboration with other nursing staff to coordinate and provide direction of care during weekday operations.
- Serves as coordinator of care for scheduled weekend and holiday shifts.
- Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin.
- Education: Diploma from accredited program in practical nursing required.
- Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing
- Certification: Current CPR/AED certification
- Every other weekend
- Every other holiday
