Elmington Senior Living is looking for an LPN at their Creekside at Three Rivers location!!

As an LPN you will provide direct and individualized nursing care to residents. The LPN works as part of the nursing team to assess patient needs, plan and modify care and implement interventions.

ESSENTIAL JOB FUNCTIONS include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Provides direct resident care and treatments, including, but not limited to:
  • Prepares, administers and records medications in accordance with community policy.
  • Monitors and records resident vital signs, such as temperatures and blood pressure
  • Performs standard nursing duties, such as dressing wounds, treating bedsores, giving enemas, catheterizations,
  • Sterilizes equipment and supplies as
  • Prepares paperwork for laboratory requests, including X-Ray and other Collects samples such as urine and sputum, and sends to laboratory with paperwork to perform tests on samples.
  • Prepares residents for medical treatments, examinations and tests, explaining the procedures and their
  • Administers prescribed emergency care, such as, but not limited to
  • Answers residents' call-lights and determines how to assist them, as
  • Assess residents' conditions and vitals to determine and/or modify resident treatment
  • Monitors, records and reports symptoms and changes in residents'
  • Provides input for the development of individualized care plans and implements
  • Maintains accurate, detailed reports and records.
  • Documents nursing history and physical assessment for assigned residents.
  • Maintains confidentiality of all resident information and ensures resident privacy in accordance with HIPAA and community
  • Promotes and supports the greatest possible degree of independence for
  • May be required to be "on-call" periodically or as specifically needed by community.
  • Reports any issues or problems that may arise to the Wellness Director and/or Executive
  • Complies with state, federal, and all other applicable health care and safety
  • Assists families and other visitors as
  • Attends/completes required in-services and other required meetings.

SUPERVISORY REQUIREMENTS of this position are generally as follows:

  • Occasional supervision of non-licensed nursing staff at the direction of the Wellness


EDUCATION
and EXPERIENCE an equivalent combination of education, training and experience will be considered.

  • Graduate from an accredited nursing program; minimum of 1 year of previous long-term care experience; or an equivalent combination of education and experience.
  • Must be licensed in the applicable state as a Licensed Practical Nurse (LPN). Must maintain CPR/First- Aid certification.
