Job Description

Ocean Breeze Palliative is hiring qualified LVNs (Licensed Vocational Nurse) to join our team of professionals. LVNs are closely supervised to ensure their competence in providing care and reports to Intake Supervisor/Case Manager. Responsible for managing all aspects of the direct patient care and implementing the Plan of Care.

Essential Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
• Participates in coordination of palliative services, appropriately reporting the identified needs to interdisciplinary group.
• Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.
• Reviews patient charts for QAPI.

Physical Requirements:

Must provide evidence of annual TB test and other state-required tests or examinations.

Mental Requirements:

Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Working Conditions:

Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.

Transportation:

Must have a current valid driver’s license, auto liability insurance and reliable transportation

All PPE provided and expected to be worn at all times.

Job Types: Part-time, Per diem, Travel nursing

Salary: $40.00 - $45.00 per hour

• Palliative care: 1 year (Preferred)
• Knowledge of Joint Commission accreditation requirements (Preferred)

Language:

• Spanish (Preferred)

License/Certification:

• LVN/LPN license (Required)
• BLS Certification (Required)

Willingness to travel:

• 50% (Required)
