Maunalani Nursing and Rehabilitation Center is seeking a Licensed Social Worker to join our team. The Social Worker, plans and coordinates social work to assure that residents meet/maintain their highest practicable emotional and psychosocial well-being.

Responsibilities and Duties

· Coordinates and participates with all department managers in developing a strong social services processes amongst team members.

· Perform and monitor residents behaviors and psychosocial needs and addressing concerns with family members as needed.

· Completes and maintains social service assessments and care plans for each resident by inputting in MDS.

· Helping patients on a daily basis to adapt to their limitations

· Coordinates care with members of the interdisciplinary team.

· Assists with admissions and discharges.

· Oversee PASRR compliance with all new admissions and as needed in-house.

Required Qualifications:

· Master's degree in social work (MSW)

· Current State of Hawaii license to practice social work (LSW)

Preferred Qualifications:

· Experience working with older adults or in long-term care.

· Prior Skilled Nursing Facility experience preferred

Job Type: Full-time

Pay: $63,000.00 - $75,000.00 per year
