Purpose of Position/Summary:

A Licensed Practical Nurse performs nursing care of patients with physical and/or mental disabilities.

Essential Duties/Responsibilities:

Administers medications or treatments prescribed by a physician;

Observes patients/residents and keeps accurate patients’ records concerning vital signs, etc.;

Contacts physicians and takes orders in the absence of a professional nurse;

Assists in specialized programs for patients such as re-motivation groups, sensory stimulation and behavior modification after completion of designated in-service training program;

Assists physicians in physical examinations of patients/residents;

Orders supplies and medicines;

Job Requirements:

Must have and hold a valid LPN license from the State of Indiana;

Specialized knowledge of principles of nursing care and ability to apply them;

Working knowledge of sanitary conditions, proper housekeeping and personal hygiene;

Working knowledge of patients’/residents’ behavior and its meaning;

Ability to maintain patients’/residents’ records and prepare reports;

Ability to maintain good relationships and direct work of other paraprofessional and nonprofessional staff;

Effectively communicate, both orally and in writing;

Ability to safely set up and administer medications and treatments.

This role does not provide direct supervision to direct reports.

Work is of moderate complexity requiring the application of specialized knowledge to relatively standardized tasks.

The LPN works under the general supervision and direction of a physician and registered professional nurse in accordance with prescribed policies and procedures. Guidelines consist of federal and state nursing practice laws, established techniques, established hospital policy and procedures. This individual exercises independent judgment following general or detailed instructions depending upon the task. Errors are not readily detected and can cause substantial discomfort to patients or residents.

This individual will be expected to partner with agency personnel and related stakeholders working towards completion of assigned functions. Exceptional customer service is provided to agency partners and/or the general public.

A high level of judgement and effective communication is required during times of contact with facility residents/offenders.

Partnership with relevant public agencies, federal, state & local governments and/or community leaders may be required.

Facility residents or offenders may have family, friends, community partners and/or other interested parties come to the facility requesting information or to visit with the resident according to policies and procedures.

Depending on the duties performed or environment setting, frequent walking, standing, climbing stairs or ladders, bending, stooping, pushing, pulling and/or similar movements may be involved.

This role requires above average strength when restraining one or more patients or offenders as per departmental policy and procedure.

This role requires the ability to lift patients and push wheelchairs in some cases.

This role requires legible handwriting to help partners in understanding written communication.

This role requires the ability to run short distances to provide immediate care, protection, or attention to patients or offenders.

This role requires keen observational and hearing skills to stay alert for possible dangerous situations. This role performs work in a state operated hospital facility.

Operations may require this role to be available after hours (on-call) and/or work rotating shifts, evenings and weekends, or irregular hours.

There is potential for exposure to infectious diseases in this role.

This role may experience periods of high stress and/or pressure.

This role requires the appropriate use of Personal Protective Equipment (PPE) when necessary.
