About the Job:
Patriot Home Care requires a caring and compassionate nurse to work with a variety of patients and clients in the surrounding areas. The
LPN will be responsible for compiling the accepted professional standards and principles. The nurse is responsible for providing in-home care for the elderly and ailing who need ongoing attention, yet do not require a hospital stay. Your dues will require you to be patient, compassionate, and have a wealth of knowledge of diseases, symptoms, and conditions of the patient and clients. The candidate
must be local to the
Wilmington, DE area. All Nurses must have a
clinical license from the
state of Delaware or a
multi-state license.
Responsibilities:
- Licensed Practical Nurse-home care administers nursing care in-home care environment under the supervision of the registered nurse or another medical supervisor.
- Provides basic medical care, including changing bandages, administering medication, and collecting specimens.
- Monitors and reports changes in patient's condition to supervisor typically reports to a registered nurse or supervisor.
- Dues include obtaining, reading, and comprehending physician orders, faxing, emailing, and interacting with patients, clinicians and doctors.
- You will coordinate care delivery which will ensure patient health, safety, and welfare in accordance with professional standards of practice through physician orders, agency policies, and procedures, and federal state, and local guidelines.
- You will obtain copies of social security cards and IDs if available and submit them to the branch business office manager if not already submitted.
- You will determine the ongoing eligibility for home health care. In addition to insurance, patients must be homebound to receive our services.
