Serenity Hospice Care has an immidiate opening for a Licensed Social Worker In Mercer/Burlington Counties to perform the social services section of the assessment process; including but not limited to;

• Completing the psych-social assessment
• Educating the patient and family about the Hospice benefit and gathering financial information.
• Develops a plan of care with the interdisciplinary team, the patient and the family to deal with personal, financial and environmental difficulties experienced by the patient.
• Assists the Interdisciplinary team to understand the significance of social, emotional and financial factors related to the patient's care.
• Assesses and reassesses social, emotional and financial factors in order to help the patient and family cope with problems related to the patient's lie limiting illness.
• Identifies and utilizes community and family resources to assist with the patients plan of care. Develops, prepares and maintains clinical documentation with accuracy, timelines and according to prescribed policies.
• Must be an LSW in the state of NJ.
• One year hospice experience
• Willing to travel
• Be familiar with the territory
• Hours 8:30-4:30 Monday thru Friday.
• No nights or weekends required.
• Must work independently but also work well within an Interdisciplinary team.
