Develops a comprehensive social history and psychosocial assessment that includes the resident's problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.
Orients patients and families to services and care plan
Involves patients and families in treatment planning by identifying measurable goals
Monitoring of treatment plan with the patient on a daily basis
Completes mandatory documentation within timeliness established by applicable regulations.
Must have 3 years of experience on all forms of Dementia
Job Type: Full-time
Pay: $65,000.00 - $70,000.00 per year
