Totally Kids Rehabilitation Hospital has a position available for a Social Worker Assistant/Service Coordination.

JOB SUMMARY

The Social Worker Assistant/Service Coordinator under the direction of the Case Manager Supervisor and/or Licensed Clinical Social Worker (LCSW) is responsible in assessing, providing, coordinating and monitoring healthcare services for all assigned patients. Ensures the medical appropriateness criteria (medical necessity) met for level of care provided in the acute and subacute units and documented from admission through discharge including readmissions. Coordinates and participates in patient discharge teaching as appropriate.

The Social Worker Assistant/Service Coordinator acts as a liaison between the patient, family members, interdisciplinary team and community to assure that services provided meet the patients needs are initiated and met in accordance with their plan of care and treatment. Ensures medically related emotional and social needs of the patient are met and/or maintained on an individual basis. Acts as a community liaison with hospital staff, insurance companies, other payors, physicians, and other providers to inform of services offered. Participates and communicates with billing department in obtaining authorization for admissions, services provided, and outpatient services. Coordinates and participates with Marketing on any vendor marketing activity representing Totally Kids Rehabilitation Hospital.

REQUIREMENTS:

  • Bachelors of Science degree in social work (BSW) required
  • Current basic cardiac life support (BCLS) certification
  • Minimum of (2) years of acute or subacute social worker assistant experience. Social work experience of the medically complex and/or in pediatric care preferred
  • Knowledge of current psychosocial issues within the community and healthcare environment
  • Must possess good work ethics and a professional image at all times
  • Knowledge of CMS conditions of participation, CDPH state regulations, CCS, and TJC (hospital, nursing care center, and laboratory) accreditation requirements as applicable to job
  • Working knowledge of computer and software applications as applicable to job
  • Possess strong communication, interpersonal and collaboration skills
  • Works responsibly in a team environment as well as independently
  • Manages confidential information effectively and appropriately

SPECIFIC FUNCTIONAL RESPONSIBILITY:

A. Service Coordinator

  • Evaluates and screens potential admissions to the facility when appropriate
  • Coordinates patient admission process with the interdisciplinary team
  • Collaborates with Clinical leadership and Director of Staff Education for educational needs of patients and families prior to admission as needed
  • Initiates and coordinates the patients individualized interdisciplinary care plan with a discharge plan that is comprehensive and best meets the continuing health care needs including follow-up services (i.e. durable medical equipment, home health, and/or placement). Reevaluates patient plan of care and ensures continued appropriateness based on the patients changing needs and condition
  • Directs and coordinates the patients plan of care conferences involving patient/family, nursing and other interdisciplinary team members, as required by hospital policy and/or regulation.
  • Facilitates effective and efficient patient care during the hospital stay. Interacts professionally with patient/family for comprehensive assessment for discharge needs taking into account the patients needs, current and prospective functional capacities, follow-up services (i.e. durable medical equipment, home health, and/or placement), resources, and support systems
  • Documents timely regarding patients problems, goals, plans, and/or social service interventions
  • Maintains current knowledge of resources available within the community and supply of resource materials to distributed to patients when needed
  • Coordinates and provides tours of the facility for potential admissions and/or community agencies

B. Social Worker Assistant

  • Acts as a liaison between the patient, family members, the interdisciplinary team and community to assure that services to meet the patients needs are initiated and mete in accordance with their plans of care and treatment
  • Coordinates with local or state agencies and programs providing services to children, such as Regional Center, Child/Adult Protective Services (CPS/APS), California Childrens Services (CCS)
  • Collaborates with case management the coordination of patient and caregiver training
  • Coordinates with case management the scheduling of patients appointments and arranges for transportation
  • Schedules on-site appointments for psychological services as needed by patients
  • Documents activities related to social services, discharge planning, community referrals, and coordination of patient services
  • Participates in the development and implementation of Rights and Responsibilities policies, guidelines and forms
  • Demonstrates an ability to be flexible, organized, and function under stressful situations

C. Support to Licensed Clinical Social Worker (LCSW)

  • Assists the Licensed Clinical Social Worker (LCSW) in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this hospital
  • Completes psychosocial assessment for each patient and identifies social, emotional, and psychological needs
  • Assists with suicide screening assessment according to hospital policy
  • Provides support and education to patients and their family members to assist in short or long term placement and referring them to the appropriate social service agencies
  • Provides interventions to address events that occurs during inpatient hospital admission
