Responsibilities:

Horizon Health, the nation’s leading psychiatric contract management services, in partnership with Livingston Regional Hospital in Livingston, TN (21 miles from Cookeville; 29 miles from Jamestown; and 41 miles from Croville is seeking a full-time Intake Services Coordinator for its 16-bed geriatric behavioral health program.


Responsibilities include all aspects of patient assessment for admission into the inpatient behavioral health unit(s). Includes ongoing concurrent review of inpatients with payors and community education with hospitals and nursing home facilities. Develops and implements referral development and pre-admission for each patient, demonstrates knowledge of regulations influencing admission and discharge plans, and post-hospital care and participates in data collection efforts for quality assurance and quality improvement purposes.

Qualifications:
  • RN or LSW
  • LCSW, LPC or LMFT licensure preferred
  • Previous experience assessment experience preferred
  • Experience in psychiatric utilization review and inpatient psychiatric treatment preferred
  • Proficient in software applications
  • Effective presentation skills
  • Excellent written and verbal communication and presentation skills
