ChristianaCare HomeHealth is seeking a Full Time Intake Coordinator. The successful candidate will work out of our office in Wilmington/Avenue North working Monday through Friday, weekend and holiday commitment are required based upon business need. The successful candidate will be responsible for working with liaison staff to obtain complete admission/financial information on newly referred patients to HomeHealth. The role is responsible to assure that the admission file information is complete and accurate to facilitate patient care and billing.

Duties and Responsibilities:

  • Ensure documentation is accurate, complete and timely in order to successfully meet federal and state regulations
  • Identify and enter the correct Diagnosis coding to receive maximum reimbursement for services
  • Review accuracy of fields on the plan of care based on CMS Medicare Conditions of participation
  • Run reports to ensure plan of care and OASIS documents are completed timely by clinicians for review and are locked based on organizational benchmarks and regulatory requirements
  • Report trends of non-adherence to providing needed data for coding, complete and timely submission of documentation to manager
  • Researches and corrects clinical data entry errors; report compliance concerns related to federal and state guidelines immediately to manager or Corporate Compliance

Education and Experience Required:

  • High School Diploma or GED.
  • Ability to generate reports, excel experience/knowledge.
  • Working knowledge of health care delivery systems and understanding of insurance coordination of benefits.
  • Exceptional customer service skills.
