POSITION: County Caseworker 1 (Housing Navigator Caseworker)

SALARY RANGE: $18.80/hr

UMWA Union Position

HOURS: 35 Hours per Week - 8:30 am to 4:30 pm – Monday to Friday

OVERALL OBJECTIVE OF HOUSING COORDINATOR JOB:

To provide assessment and case coordination to persons/families that may have forensic or substance abuse concerns and that are homeless or at risk of being homelessness that assists the person/family with the goal of improving their housing stability.

ESSENTIAL FUNCTIONS OF HOUSING COORDINATOR:

1. To complete intakes of clients who need housing services and by collecting the data and paperwork assigned by funder and or program.

2. To identify clients who are homeless, unsheltered and sheltered through direct outreach activities while also receiving referrals from shelters, police, churches, human services providers and others.

1. Complete and document engagement efforts, including but not limited to requirements set forth by grantors, such as Coordinated Entry Assessment, Fair Housing Assessment, PATH assessment, etc.

2. Deliver outreach which my occur in a community setting where literally homeless persons frequent such as shelters, churches, soup kitchens, Point In Time Count, special events and including the streets.

To provide screening, housing assessments and eligibility determination to persons who meet the target population of the State Opioid Response Support Services Navigation & Housing Services for Individuals with Opioid Use Disorder grant, homeless or in imamate risk of being homeless, or/ and have a housing issue.

Will need to specialize in working with and directly supporting those with OUD.

Will attend trainings that will assist in helping those in the program navigate the Drug and Alcohol Recovery system.

Coordinate MDT meetings when necessary to provide cohesiveness with other appropriate levels of care, organizations and providers that consumers may need to become successful. This is to include the court system.

To provide housing case management services, including coordination of services to persons who are enrolled in a housing related program on an individualized basis. Coordination of these services will touch on all social determinants, transportation, housing food, etc.

Work with landlords to assure our consumers have a voice and be available to provide conflict resolution as need be.

Maintain all files with required documentation required, including client case record activity and progress, by the guidelines set forth by each funding stream and in regard to required timelines.

Data entry of all pertinent data into HMIS/ HAP database and other data bases that may be required from a funding source.

Complete all monthly, quarterly, and annual reporting required by all housing funding streams.

Works as a liaison with referral agencies and providers to assure clients receive a maximum service level for success.

Attends all meetings related to housing as assigned.

Maintain case file information and documents to ensure compliance, work quality and confidentiality.

Maintain confidentiality based on Federal, State and Department standards.

OTHER DUTIES OF JOB:

Utilizes computer and software programs and other office equipment as needed to carry out job duties.

4. Assists other workers in meeting the needs of the consumers

5. Performs all other duties as assigned.

SUPERVISION RECEIVED:

Receives instruction and supervision in regard to daily work duties from the Housing and Family Resources Program Director.

SUPERVISION GIVEN:

None

WORKING CONDITIONS:

1. Works indoors in adequate office space, lighting, ventilation and temperatures.

2. Normal exposure to noise and stress, but subject to frequent disruptions.

3. Normal exposure to indoor dust/dirt.

PHYSICAL/MENTAL CONDITIONS:

1. Must possess ability to record, convey and present information, explain procedures and follow instructions.

2. Must be able to sit for long periods throughout the workday, with intermittent periods of standing and walking and occasional twisting, bending, stooping, reaching as necessary to carry out job duties.

3. Dexterity requirements range from coordinated movements of fingers/hands for typing and computer, to simple dexterity of feet/legs; torso as necessary to carry out job duties.

4. Sedentary work, with occasional lifting/carrying of objects with a maximum weight of ten pounds.

5. Must be able to pay close attention to detail and to concentrate on work.

QUALIFICATIONS:

A. EDUCATION/TRAINING:

A bachelor’s degree with a social work major and 2 years of direct social service experience;

or a bachelors degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other relates social sciences and 2 years of direct social service work Experience.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

1. Must be able to speak and understand the English language and to follow oral and written instructions.

2. Must possess knowledge of current social case management principles, techniques and methods.

3. Must possess knowledge of current social, economic, and health problems and resources and the recognition of their impact on the growth and development of people.

4. Must possess knowledge of individual and group development and behaviors and ways of working effectively with adults and children who have social, economic, emotional, or health problems.

5. Must possess knowledge of the basic principles and methods of program interpretation and community organizations.

6. Must possess the ability to work effectively with people and aid them to grow in the constructive use of their potential in adjusting to their specific disability.

7. Must possess the ability to understand and assess the needs and rights of others and to work with adults and children who are physically challenged, emotionally troubled, or economically disadvantaged.

8. Must possess effective oral and written communication skills.

9. Must possess initiative and problem solving skills.

10. Must possess the ability to establish and maintain effective working relationships with representatives of local, state, and federal agencies; health organizations and institutions; and the general public.

11. Must possess the ability to perform research and organize professional presentations.

12. Must possess the technical knowledge of operating personal computers and other office equipment.

13. Must possess good communication skills, both orally and in writing.

14. Must have ability to prepare required reports in a timely and accurate manner.

15. Must possess knowledge of federal, state and county laws, policies and procedures in regard to operating local government, county code and administrative policies and procedures.

16. Must possess ability to handle a high volume of work efficiently.

17. Must possess basic math skills and ability to accurately prepare required reports and mathematical record keeping.

18. Must possess ability to maintain confidentiality in regard to county information and files and records.
