Social Worker for Hospice
The Social Worker for Hospice provides assessments of the psycho-social needs of patients and family. Works closely with both the Hospice and Home Care departments to implement a comprehensive plan of care in the home environment. HCS cares for those in the Keene, Charlestown and Peterborough, NH regions.
***This position offers a $5,000 Retention Bonus, paid in quarterly installments of $1,250***
This is a full-time 37.5-hour per week position worked Monday – Friday. Responsibilities include, but are not limited to:
  • Assisting in developing and implementing a plan of care to meet the psycho-social needs of clients;
  • Actively engaging in regular, individual and group support to all social work staff;
  • Knowing and using community resources;
  • Participating in education programs for staff and community;
  • Assisting with the orientation of new staff;
  • Seeking continuing education programs to upgrade skills and maintain license where applicable; and
  • Conveying a positive image of the agency through personal appearance, attitudes, and behaviors.
Qualifications:
  • three (3) year or more experience in a healthcare environment;
  • experience with death, dying, grief, palliative care, and bereavement in the field of social services preferred;
  • Bachelor’s degree in human services related field required; Masters in Social Work degree preferred
  • valid driver’s license and vehicle insurance; and
  • must be able and willing to learn to utilize a portable electronic field device to manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures.
