Homeless Case Manager

SUMMARY: Case Managers maintain responsibility for a caseload and provide case management services for clients who have been diagnosed with a serious mental illness and are homeless or at risk for homelessness.

In addition to housing, the case manager assists with other case management needs of the individual. The case manager works in a team setting and participates in a multidisciplinary team meeting and receives clinical supervision. The case manager receives training in the HMIS (Homeless Management Information System) and the VI-SPDAT (Vulnerability Index - Service Prioritization Decision Assistance Tool), and complete all other required documentation including a Housing Service Plan. Will conduct street outreach, meeting homeless individuals in community settings. The case manager develops excellent working relationships with community partners, housing management, and landlords.

DUTIES:

- The case manager completes required assessments and other needed documentation which includes the Housing Service Plan, releases of information, progress notes, and referrals to internal and external providers.

- The case manager arranges meetings with client and other providers and coordinates needed client services in areas of housing, mental health, chemical health, financial benefits, insurance benefits, employment, transportation, and other basic needs.

- The case managers work cooperatively with other service providers and has a good understanding of program eligibility requirements and referral aspects associated with community resources.

MINIMUM QUALIFICATIONS:

- 2000 hours work experience working with individuals who have a mental illness, homeless, and have high barriers to sustaining housing is desired.

- 6000 hours of related work experience with some college will be considered.

- Reliable transportation, proof of vehicle insurance, and valid driver's license.

- Previous experience in providing housing services preferred.

DESIRED QUALIFICATIONS:

- Ability to work in a team setting and knowledge of community resources.

- At least 2 years working with diverse individuals who have been homeless or at risk for homelessness.
