We are seeking an Occupational Therapist - Home Health Per Diem to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients in a home health setup. Let us know which areas you can service and we'll match you with the right referrals!

Please note that our rates are per visit/case basis.

Responsibilities:

• Participate as a member of the interdisciplinary team providing support and information within the area of Occupational Therapy.
• Evaluates patients referred for home care services to determine appropriateness for physical therapy services.
• Establishes a home care plan including assessment of the appropriateness of the requested services.
• Completes documentation in a timely manner
• Makes initial evaluation visits to patients and develops/implements an appropriate Plan of Care based on skilled assessment and various tests to determine patient's individualised needs.
• Assists in the supervision of a COTA
• Maintains and submits appropriate documentation, notes and reports

Qualifications:

• Previous experience in occupational therapy or home health
• Ability to build rapport with patients
• Strong problem solving and critical thinking skills
• Ability to thrive in a fast-paced environment

• Professional License

• Auto Insurance
• Active CPR or BLS Certification
• Health Certification or Recent Physical Examination/Fit to Work note
• Valid (1 year) TB/PPD Test or Valid (3 years) Chest Xray
• Covid Vaccine record ***including booster
• Influenza Vaccine (or Declination)
• MMR
• Hepa B
• Photo
• Reference Check (at least 2 credible referees)
