Job title: Specialized Service Worker
Work on site with clients residing in nursing facilities. We are looking for a compassionate, dedicated, and reliable team member to join our Active Treatment department. This is a part time role with 15-20 hours/week.
Facilities serviced located in: Brockton, West Roxbury, Walpole
Main job functions include:
- Under the direction of the Active Treatment Supervisor, provide quality one to one support to individuals with disabilities
- Work onsite at nursing facilities in the Boston & surrounding vicinity
- Provide one to one support to individuals with disabilities to maintain skills that foster personal growth and independence
- Assess needs of your clients and support with developing care plans
- Assist clients carry out therapeutic goals
- Provide opportunities for community reintegration
- Document client progress
- Work with clients multidisciplinary team, including other service coordinators
Great position for someone seeking hands on social service experience.
Must have your own transportation. Mileage and travel in between facilities are reimbursed.
COTAs, CTRSs, CNAs, Activity professionals or other healthcare providers encouraged to apply.
Must be fully vaccinated against COVID19.
