Saint Simons By-The-Sea Hospital is seeking a PT Discharge Planner, who serves as an active member of the clinical staff; Maintains a therapeutic relationship with all patients; Schedules appropriate aftercare appointments for patient’s continuity of care; Facilitates transition to special placements such as treatment centers, nursing homes, halfway houses, and group homes; Researches for newly identified programming and providers; Maintains referral database; Facilitates education group on discharge planning.

Responsibilities include, but are not limited to, the following tasks:
  • Collaborates with the physicians, therapists, nurses, and other members of the treatment team as well as patients in order to develop effective continuing care plans for patients.
  • Provides patients with reasonable aftercare options and discusses options with the patient individually and/or with family, as requested by the patient.
  • Collects releases of information to accurately research and refer to services.
  • Maintains timely and accurate documentation of discharge planning efforts.
  • Initiates contact with referral sources such as community mental health providers, specialty physicians, nursing homes, residential treatment facilities, halfway houses, and other placement options to facilitate continuity of care.
  • Coordinates admissions to secondary hospitals, treatment centers, nursing homes, halfway houses, and other placement as needed.
  • Schedules follow-up appointments with psychiatrist, primary care physician, and therapist within outlined timeframes, whenever possible (MD: 15-30 days; Therapist: 7 days).
  • Attends and contributes to daily treatment team meetings in a punctual and professional manner when needed for collaboration.
  • Maintains positive relationships with community resources/referrals and represents the facility in a professional manner.
  • Actively researches and updates treatment team on newly identified referral sources for various levels of care.
  • Facilitates educational groups to patients on discharge planning and document accordingly on progress notes.
  • Maintains referral, contact, and resource database.
  • Data entry into Midas format regarding patient aftercare.
  • Responds to any emergency situation involving patients in treatment.

Qualifications:
Requirements:
  • Bachelor’s degree in Social Sciences or related field is required
  • Knowledge and experience of confidentiality and compliance with HIPAA regulations
  • Basic knowledge and experience of mental health and substance abuse issues in a variety of patient populations
  • Effective time management and ability to work under stress
  • Excellent written/verbal communication skills
